FELLOW COMMUNITY MEMBERS:
LAKE COUNTRY PHYSICAL THERAPY & SPORTSCARE, PC HAS REOPENED FOR AT OFFICE INITIAL AND FOLLOW UP VISITS IN ADDITION TO TELEHEALTH CARE.
All necessary COVID19 precautions will be taken, before, during and after your appointment. Please arrive and wait in the vestibule for your greeting.
Physical therapy is considered essential health care, the office was closed for non-emergent care March 20 – May 5, 2020 to protect you and the wellbeing of the PT team.
Telehealth services are STILL FREE and available to you UNTIL THE STATE OF EMERGENCY IS LIFTED. Emergency Physical Therapy visits require a physician’s referral.
Most Insurances are now covering Initial and follow up visits. If you do not have insurance coverage, during the state of emergency, you can initiate care at a reduced rate of $85.00 per initial visit and $50.00 per subsequent sessions.
Telehealth Visits for all insurance companies are covered at no cost to you.
What you need to do:
- Make an appointment by calling 585-396-1400.
- We will help you access ZOOM, or What’s app
- A telephone visit is also an option
- If you choose my preference of ZOOM then, I will send you an email or a text with a link to click on and access codes for you to enter. You do NOT need to download the program to join me for your physical therapy session.
- If you choose What’s app you need to have the program on your phone. I will call you at your scheduled time.
On March 16, 2020 Zoe~nergy yoga was closed. I am considering YOGA with ZOE on ZOOM at no cost with optional donations via Paypal. PLEASE EMAIL ME IF YOU ARE INTERESTED. Zoe@lakecountrypt.com
