MISSION STATEMENT

In 1987, Lake Country Physical Therapy and Sportscare, P.C. initiated a strong commitment toward providing the Canandaigua community and surrounding counties appropriate and cost effective physical therapy services.

In the midst of the challenging changes in health care, the facility continues to maintain its reputation of being a personalized yet professional medical practice. Our relaxed and informative approach to small business functions displays a genuine concern for the consumer.

It is our goal to continuously strive toward satisfaction of the consumer, the referral source and the third party payers.

Since the inception of the private practice, the team goal has been to provide progressive pain management and rehabilitation programs. It is our ongoing mission to evaluate the preventive medicine needs of the health care consumer and to offer new and existing services in a convenient fashion.

The primary mission is to educate the participant as to their responsibility in pain control, enhancement of functional capabilities and in the return to desirable activities, sport, recreational and work status.

Each team member is employed through commitment and drive toward professional care and growth.

ORGANIZATION

Lake Country Physical Therapy and Sportscare, P.C. is an outpatient, privately owned, Professional Corporation in Canandaigua, New York. Situated in the hub of the Canandaigua medical community, the office is conveniently located in the Parrish Medical Park on Parrish Street.

Lake Country Physical Therapy and Sportscare, P.C. was established in 1987 and is owned and operated by Zoe Fackelman, PT. Our team also includes a practice manager, a cleaning person, a maintenance person and a snow and lawn care service.

The facility provides physical therapy pain management for acute and chronic injuries and conditions, rehabilitation protocols for all types of spine and extremity surgeries, test and measurement evaluations to determine levels of function for injured workers and social security disability applicants, preventive and ergonomic education programs to community members, small businesses, athletic teams or industry.

Specialty services include: hands-on treatment, spine care, pregnancy care, continence care, pelvic pain as outlined in the Services section of the website, osteopenia and osteoporosis care, postural correction ,TMJ and golf performance enhancement program and injury rehabilitation.

Our 2500 square feet facility houses a variety of physical therapy modalities to expedite pain relief and restoration of movement. We have developed self pay programs for facility use by the community for continued wellness. The view of Canandaigua Lake and our surroundings of blue and green offer a relaxed yet motivating environment for the use of our state of the art exercise and testing equipment.

The professional corporation is guided by a network of advisors including, American Physical Therapy Association, National Strength and Conditioning Association, National Association for Continence, International Pelvic Pain Society, Canandaigua Chamber of Commerce, Canandaigua Business and Professional Women's group and the Canandaigua Rotary.

PRACTICE PHILOSOPHY AND PERFORMANCE STANDARDS

The services available at Lake Country Physical Therapy and Sportscare, P.C. benefit individuals with physical limitations as a result of orthopedic, sports, motor vehicle or work related injuries and men's and women's health concerns.

Early intervention speeds recovery. Therefore, our scheduling is designed to begin care within two days of inquiry and no later than five days. Personnel at every level are encouraged to make decisions consistent with their level of competency, and to seek advice of others in areas that require the expertise of a specialist.

The evaluation and treatment approach emphasizes pain reduction, restorative motion, functional outcomes and adequate retention of self and preventive care principles. The process is initiated with instruction of a home program. Each session, the home program is progressed according to the outcomes of the physical therapy session. All personnel involved with the health care of the consumer are trained in manual therapy skills and possess a commitment toward patient education.

The success of Lake Country Physical Therapy and Sportscare, P.C. can be attributed to the belief that the consumer is the key advisor to the rehabilitation team. The rehabilitation team also includes the physician, the health insurance representatives, and the consumer's employer. It is our practice to educate and involve the consumer in the responsibilities of their recovery from an illness, injury or disability. Participation in all stages of rehabilitation is mandatory. If noncompliance is observed, the consumer is encouraged to reconsider their investment in the health care system.

Continued quality and progressive care is monitored through biweekly staff meetings, weekly performance enhancement sessions, annual staff performance reviews, patient and physician satisfaction surveys and ongoing public relations with the insurance representatives and rehabilitation specialists. Policy and procedure manuals for both the clinical setting and the business office are the training guides for all interactions.

Industries, small businesses, community groups, or clubs benefit from our consulting services in the instruction of fitness programs, ergonomic assessments or injury prevention programs.

Each team member is offered the benefit and is encouraged to participate in continuing education programs in order to strive toward excellence in professional care and growth.
